Food Allergy Information
MEAL SUBSTITUTIONS FOR MEDICAL OR SPECIAL DIETARY REASONS
USDA Regulation 7 CFR Part 15b requires substitutions or modifications in school meals for children whose disabilities restrict their diets. A child with a disability must be provided substitutions in foods when that need is supported by a signed statement from a licensed physician.
For a student to "opt out" of sitting on the allergy table for the 2017/18 school year, this form needs to be completed and returned to the school nurse.
Student Health Collaboration
Delaware school districts signed up their nurses and I am now participating with Nemours so I can connect better with my students at NSE. The program that makes this possible is called Nemours Link.
Your input as parents is very important and I encourage you to stay in touch with me over the school year regarding changes in medicine, or anything else you feel is important in regards to your child’s care plan. Nemours Link is a secure way for Nemours providers to share medical information with school nurse’s who are considered community medical providers.
As your child’s community medical provider at North Star Elementary school my nursing care can be enhanced by viewing your child’s health record and assisting you with obtaining for example, notes from your physician, an asthma or food allergy action plan or lab reports.
